Empress Shed and Ascend
Empress Shed & Ascend is our medically supervised weight management program, built around GLP-1 and dual GLP/GIP medications — semaglutide and tirzepatide. These medications mimic naturally occurring hormones that help regulate appetite, slow gastric emptying, and stabilize blood sugar. The result is that you feel full sooner, stay full longer, and experience fewer cravings, without stimulants or severe restriction. The right medication for you is chosen at your consultation. As your dose is gradually titrated upward, most patients see steady, sustainable progress alongside improved energy and digestion. Treatment is prescribed and monitored throughout.
HOW IT WORKS
Semaglutide is a GLP-1 receptor agonist, and tirzepatide is a dual GLP/GIP receptor agonist. Both mimic hormones your body produces naturally to regulate hunger, satiety, and blood sugar. Given weekly, they act on the appetite centers of the brain so that fullness registers sooner and lasts longer. They also slow gastric emptying, which keeps you satisfied for longer and reduces the urge to snack or overeat. At the same time they help stabilize blood sugar and blunt insulin spikes, which for many patients means steadier energy and fewer cravings. The result is gradual, sustained weight loss without restrictive dieting.
WHAT TO EXPECT
Treatment starts at a low dose so your body can adjust, and the dose is increased gradually based on your response. In the first few weeks most patients notice reduced appetite, fewer cravings, and earlier fullness at meals. Weight loss generally begins around week 2 to 4 and averages one to three pounds per week. Many patients also report steadier energy, better focus, and more stable mood. Mild side effects such as nausea, fatigue, or changes in digestion can occur with the first few doses and usually ease as your body adapts. You are monitored throughout, with dose adjustments as needed and guidance on nutrition, hydration, and activity.
FAQs
How much treatment will I need?
Treatment is individualized based on your goals, your metabolic health, and how you respond to the medication. Most patients commit to at least 12 weeks to see sustainable change in both weight and eating habits. Dosing begins low and is increased gradually according to your response and tolerance, which minimizes side effects and supports long-term results. Once you reach your goal weight, we move to a maintenance or tapering phase. Some patients continue on a low maintenance dose, while others discontinue treatment entirely.
Does it hurt?
Empress Shed & Ascend is given as a weekly subcutaneous injection using a fine insulin-style needle, usually into the abdomen or thigh. Most patients describe it as a brief pinch. If you are sensitive to needles, a cooling compress can be applied. Injections are given once weekly, and I will either teach you to self-administer at home or give them in the office, depending on your preference.
What's the Downtime?
There is no downtime with semaglutide or tirzepatide, and you can return to normal activity immediately after your injection. Because appetite and digestion change during treatment, I recommend regular balanced meals, consistent hydration, and moderate activity. Mild side effects such as nausea, fatigue, or changes in digestion can occur in the first few weeks and usually resolve as your body adjusts.
How long do results last?
Results are durable when the medication is paired with consistent nutrition, hydration, and activity habits. Once you reach your goal weight, we move into a stabilization phase, adjusting your dose or transitioning to a low-dose maintenance plan if appropriate. Many patients maintain their results after completing the protocol, particularly when the habits built during treatment are sustained. Some weight regain is possible after stopping, which is why your maintenance plan is discussed before you finish treatment.
Are there any side effects?
Side effects are most common early in treatment and are usually temporary. They can include mild nausea, decreased appetite, fatigue, constipation, and headache. Starting at a low dose and increasing gradually reduces their severity. Less commonly, patients experience vomiting, gallbladder irritation, or more pronounced changes in digestion, which is why a full consultation and medical history review is completed before starting. Semaglutide is not appropriate for everyone, including patients with a personal or family history of medullary thyroid carcinoma or MEN2. You are monitored at each visit and your dose is adjusted as needed.
What do I need to do before my appointment?
Stay well hydrated in the 24 to 48 hours before your appointment. Eat a light meal containing protein and healthy fat on the day of your appointment to reduce the risk of nausea. Avoid alcohol, excessive caffeine, and intense exercise the day before.
What do I need to know post treatment?
Continue drinking plenty of water after your appointment, as hydration supports digestion while your appetite changes. You will likely feel full sooner, so eat slowly and stop when you are comfortably full. Mild nausea, fatigue, or lightheadedness can occur early in treatment and typically eases with time. Avoid alcohol and rich or greasy foods for the first 24 to 48 hours. Gentle activity such as walking is encouraged. Contact the office if you have persistent vomiting, severe abdominal pain, or signs of dehydration.
